For new patients, we suggest that you print, complete, and bring your new patient forms with you to your appointment, so we can expedite the process and proceed with your consultation. In addition, please bring other relevant medical and diagnostic documentation, including x-rays, MRIs, and any accompanying reports.

What To Expect During Your First Appointment
Your healthcare practitioner will inquire about your past medical history before conducting a comprehensive medical exam. It is important that we get the most accurate information regarding any past treatments (surgery, massage, acupuncture, etc.) you have received, as well as any nutritional supplements or medications (over the counter, prescription pharmaceuticals, vitamin, minerals, herbs, etc.) you have taken or are currently taking. Additional tests may be recommended before a diagnosis is made. Once the diagnosis is confirmed, our team of healthcare specialists will create a personalized treatment program that targets your specific condition and needs.
During your appointment, we encourage you to voice any concerns, symptoms, and objections you may have. Communication is extremely important, especially when it comes to determining the proper diagnosis and treatment. Though omission can happen by accident, it can influence the effectiveness of your therapy. As your healthcare provider, our concern is in providing the most effective treatment for your condition, which we cannot deliver if the information we receive is incomplete or inaccurate.
Your full participation is important for optimal treatment results. While we do what we can to create a treatment schedule that conforms with your availability, it is imperative that you maintain your commitment by attending all agreed upon appointments for the benefit of your health.
